Accessing the Form:
If you have started a form and need to add another mileage record:
Option 1:
- Go to: https://lff.d93.k12.id.us/Forms/Home/Tasks - (You will be required to log in. Go here for login help.)
- Select "Open Tasks" in the left menu:
- Click "Return to Initiator" in the tasks panel:
Option 2: If you received an email notification when you submitted your form, subject line "Mileage Reimbursment Request - In Process," there is a link at the bottom of the email that will take you directly to your form. You will be required to log in to access the form. Go here for login help.
**If the link in the email does not work, please try Option 1 above.**
To start a new form:
Option 1:
- Log into Laserfiche Forms: http://d93forms.com/ (Go here for login help.)
- Click "Start Process" in the top menu:
- Find "Acct - Mileage Reimbursement Request" in the process list (if needed, use the search box at the top of the list) and click the Start button.
Option 2: Go to https://lff.d93.k12.id.us/Forms/mileage (Go here for login help.)
Using the Form:
- Employee data fields (e.g., name, school/department, etc.) will populate automatically.
- Enter the Funding Source.
- Choose how you would like to receive your reimbursement check.
- Enter mileage information.
- Selecting from the dropdown for "From" and "To" is not required, but doing so will automatically populate the "Miles" field. Only district locations are included in the dropdowns.
- Two options for submission:
- Option 1: DO NOT check the "Submit for Reimbursement" box. With this option, when you click the "Submit" button, the form will save everything you have entered and route the form back to you for completion at a later date (see above for instructions on accessing a form in progress). This allows you to enter your mileage as frequently as necessary so you do not have to keep track of it separately and enter it all at once.
- Option 2: Check the "Submit for Reimbursement" box. This will allow you to sign and submit the form. With this option, when you click the "Submit" button, everything you have entered will be saved and the form will be sent to you supervisor for approval and from there is will be routed to Accounting to process your reimbursement.
View Completed Forms:
- Go to: https://lff.d93.k12.id.us/Forms/Home/Tasks - (You will be required to log in. Go here for login help.)
- Select "Completed Tasks" in the left menu:
- All completed forms are listed in the main pane. Click title in "Task" column to view individual forms. Please note: This will show all your submissions to save and finish later as well as your final submission for reimbursement.
